2023 Benefits Summary

Medical Plans

Cigna Traditional PPO

  • No annual deductible – in-network plan only
  • Copay is $30 for PCP and $50 for an SPC
  • Emergency room is $150 per visit
  • Inpatient copay per admission – $400 per day up to 5 days maximum
  • Prescription plan $15/$35/$50 after deductible; mail-order also at 2.5 times retail copay

HSA Information

For those enrolled in a High Deductible Plan, PSI contributes money to your Health Savings Account (HSA) to help meet the deductible. An employee with just single coverage would receive $750 for the year and an employee with spouse, child(ren), or family coverage would receive $1,500. Depending on hire date of first year of employment the payout is 50% of the ER contribution initially (i.e. Benefits effective Jan 1 will receive $375 for single and $750 for family, then 25% on Apr 15 & 25% on July 15.)

  • Hire Date between January 1st – February 1st receives 100% of the ER contribution to HSA.
  • Hire Date between March 1st – May 1st receives 75% of the ER contribution to HSA.
  • Hire Date between June 1st – August 1st receives 50% of the ER contribution to HSA.

Cigna High Deductible Plan
#1 HSA

  • Annual deductible $1,500 for individual, $3,000 for employee plus spouse, domestic partner, or child, or family
  • PSI contributes up to 50% of the annual deductible as a contribution to your HSA based on benefits effective date
  • The emergency room is covered 100% after deductible in-network only
  • Inpatient hospital plan covers 100% after deductible in-network only
  • Preventative visits are covered 100%, with no deductible
  • Prescription plan $10/$30/$50 after deductible; mail-order also at 2.5 times retail copay

Cigna High Deductible Plan
#2 HSA

  • Annual deductible $1,500 for individuals, $3,000 for employees plus spouse, domestic partner, child, or family
  • PSI contributes up to 50% of the annual deductible as a contribution to your HSA based on benefits effective date
  • Cigna In-Network Only
  • Coinsurance 90% paid by Cigna
  • Emergency room In-Network Deductible plus $200 copay
  • Preventative visits are covered 100%, with no deductible in-network only
  • Prescription plan $10/$30/$50 after deductible; mail-order also at 2.5 times retail copay

Dental - Delta Dental

  • $50 per person/$150 per family each calendar year
  • Preventive is 100% covered
  • The maximum annual benefit paid per person is $1,500

Vision - EyeMed Vision

  • $10 copay for exams in-network
  • Contact lens allowance every 12 months
  • Prescription lenses allowance every 12 months – frames every 12 months

*Please Note: Pay periods are two weeks in length. PSI Employees are paid every other Friday.

Life Insurance - Lincoln Financial

  • Benefit is paid 100% by PSI
  • Life insurance benefit is lesser 1x of your annual salary or $50,000.
  • AD&D benefit is lesser of 1x your annual salary of $50,000.

Voluntary Life Insurance - Lincoln Financial

  • Guaranteed up to $200,000 for employee
  • Guaranteed up to $30,000 for spouse
  • $10,000 for child(ren)

Medicare Choice Group

  • PSI partners with the Medicare Choice Group to offer high-touch engagement services for Medical and Individual Health coverage transitions. Advisors will provide you Medicare expertise, guidance, and enrollment support at no cost. One-to-one advisor consultations with recommendations that meet your unique needs, educational sessions, multi-carrier quote enrollment with agent assistance, compliance, and ongoing beneficiary support are available. 
  • This is an employer-paid benefit. 

Voluntary Critical Illness

  • PSI makes coverage available to you that protects against financial loss resulting from a critical illness, such as a stroke, heart attack, or major organ transplant.
  • This coverage is available at discounted rates and can be taken with you should you leave employment with PSI Pharma in the future.
  • Only offered at open enrollment.

Voluntary Accident Insurance

  • Accident coverage provides reimbursement for expenses related to surgery, emergency room visits, and hospital confinement due to an accident. 
  • This coverage is available at discounted rates and can be taken with you should you leave employment with PSI Pharma in the future.
  • Only offered at open enrollment. 

Dependent Care Reimbursment Account (DCRA)

  • Pre-tax benefit account used to pay for dependent care services while you are at work. 
  • Money contributed to a Dependent Care FSA is not subjected to payroll taxes so you end up paying less in taxes and taking home more of your paycheck.
  • The monies can be utilized for programs such as preschool, summer day camp, before or after school programs, and social and elder daycare. 

401(K) - Fidelity

  • Traditional 401(K) or Roth 401(K)
  • The eligibility date is the first day of the first month following the date of hire.
  • PSI will match 100% of the first 3% of an active participant’s compensation contributed to the plan, plus 50% of the next 2% of an active participant’s compensation contributed to the plan.
  • Matching contributions are 100% vested immediately upon deposit.

Short & Long Term Disability - Lincoln Financial

  • The benefit is paid 100% by PSI
  • Short-Term Disability: The benefit is 70% of your basic income after a 7-day elimination period up to a maximum weekly benefit of $1,500.
  • Long-Term Disability: The benefit is 70% of your basic income up to a maximum monthly benefit of $15,000.

Maternity Leave

  • This policy will supplement Short Term Disability and Parental Leave policies to ensure that all eligible employees receive 12 weeks of maternity leave total.
  • Eligible employees must: have been employed with the company for at least 12 months; have worked at least 1,250 hours during the 12 consecutive months immediately preceding the date the leave would begin; be a full-time, regular employee.

Parental Leave

  • This policy will add two weeks of paid parental leave to employees following the birth of an employee’s child or the placement of a child with an employee in connection with adoption. This policy will apply to the birthing parent and the non-birthing parent.
  • The purpose of paid parental leave is to enable the employee to care for and bond with a newborn or a newly adopted or newly placed child. The policy will run concurrently with the Family and Medical Leave Act (FMLA), Short Term Disability (STD) leave, and Maternity Leave, as applicable.
  • Eligible employees must have been employed with the company for at least 12 months.

Holidays

There are eleven fixed holidays: New Year’s Day, Good Friday, Memorial Day, Juneteenth, Independence Day, Labor Day, Thanksgiving Day, day after Thanksgiving, Christmas Eve, and Christmas Day.

Two floating holidays are available to employees hired prior to June 30.

Recharge Days

As an employee of the company, you will be eligible to receive three recharge days if you start on or before March 31. Recharge days are prorated for the remainder of the year based on an employee’s start date. 

Years of Service Awards

  • Full-time, active employees become eligible for a service award in the year they complete 3, 5, 10, 15, 20, 25, and 30 years of service. 
  • Employees will be able to select from an array of gifts in recognition of their tenure.

Note: “Anniversary” refers to the completion of the specified number of years of employment (i.e., 3rd anniversary means completion of 3 years, starting the 4th year of employment).

The Life Assistance Program (LAP), in connection with your employer-sponsored group insurance from Cigna, offers support, guidance, and resources to help you find the right balance between your work and home life. Experienced professionals are ready to provide you and your family with information, assistance, and support in several areas. 

Employee Connect - Lincoln Financial

Employee Connect offers professional, confidential services to help you and your loved ones. 

Husk Nutrition

Husk Nutrition provides insurance-funded and private pay evidence-based tele nutrition services delivered by Registered Dietitians.  

Wellness Coach

This online and app-based platform offers various wellness-related activities, including virtual fitness classes and challenges, mental wellness resources, guided meditations, and sleep-promoting audio tracks.

Cigna App

Cigna is PSI’s provider for medical benefits and offers various resources for your health and well-being, including Wellness (food, stress, sleep, exercise, weight, prevention) information as well as the ability to find care in your area and plan-specific visit costs. 

The website and app also offer virtual ID cards and other resources, including the Active & Fit Direct program, which offers discounted gym and fitness studio memberships.

Pet Insurance

Nationwide’s “My Pet Protection Plan” provides comprehensive pet insurance coverage at a group rate for your furry family members from any licensed veterinarian of your choice. You can choose from two different plans for your cats and dogs that veterinarians have designated to ensure quality coverage, with no waiting period once the policy is effective. You may also choose from two levels of reimbursement, 70% or 50%, within the My Pet Protection.
